CD4 T-helper cells are major players in adaptive immunity. Based on their expression profile of transcription factors and secreted cytokines, these cells are divided into the major subsets Th1, Th2, Th17 and T regulatory (Treg) cells. Th1 cells, induced by IL-12 and enhanced by IFNgamma, secrete IFNgamma that activates macrophages. Induced by IL-4, Th2 cells primarily produce IL-4, IL-5, IL-13, IL-25 and IL-17E, playing a role in allergy. Treg cells, induced by TGFbeta, produce the anti-inflammatory cytokines IL-10 and TGFbeta and function to control T-cell responses to prevent autoimmune reactions. Th17 cells, a more recently discovered subset characterized by the production of IL-17, are induced by TGFbeta combined with IL-23 and IL-1beta in humans, and TGFbeta, IL-6 and IL-21 in mice. Activated Th17 cells secrete IL-17A, IL-17F, IL-21, IL-22 and TNFalpha to promote tissue inflammation. Th17 cells are involved in the clearance of extracellular bacteria and fungi. They are abundant in the intestinal lamina propria and function as a barrier against invading pathogens. Excessive amounts of Th17 cells have been implicated in the pathogenesis of several autoimmune diseases, including multiple sclerosis, psoriasis, juvenile diabetes, rheumatoid arthritis, Crohn's disease, and autoimmune uveitis. In addition, Th17 cells play an important role in tumor development, progression, and metastasis, potentially in both promoting and inhibiting tumor growth.
